My name is Johnny, and I developed this book in 2012, after I lost my mum Eva who had dementia for many years.
It occurred to me upon reflection in the months after Mum’s death, that it would have been invaluable to have a journal such as this to refer to.
Indeed, to show to her visitors and carers at the nursing home, in her own words about Eva’s life and especially, her likes and dislikes.
It would also have been a valuable family keepsake.
At one point I thought it would be good to have it published, however due to a busy life and working, it just never happened.
In 2023, I retired due to some health issues and now feel it would be good to simply provide this template to those that would like to use it.
Utilising the journal can be as short or as long as you wish.
I have included some practical tips to help, but there are no rules.
I hope you enjoy the experience and that it brings you and your family some joy.

feel free to mention my name and the book.
I have not included a link because Vision Australia are easy to find online.
